Middlesex County Golden ID Cards

The Department of Human Services' Office on Aging and the Middlesex County Clerk’s office will be offering Middlesex County Golden Key ID cards.

DATE: Thursday, July 9
TIME: 10:00 a.m.
LOCATION: Jankowski Community Center - 1 Olive Street, Perth Amboy


Requirements:
1. Proof of U.S. Citizenship (U.S. Birth Certificate, U.S. Passport, or Naturalization Certificate)  Proof of legal status in the U.S. (Permanent Resident Card,  Valid Foreign Passport with I-551 Stamp and Valid Visa)
If married female, must also present marriage certificate.
2. Social Security Card
3. Proof of Middlesex County Residency
4. Fee is $6.00

Anyone interested must register with the Office on Aging in advance: 732-826-1690 ext. 4326
