The records of the Assessor are used by many government agencies for verification of ownership and mailing addresses such as 911 Services, Public Utility Services and the Health Department. It is the property owner's responsibility to advise the City of their proper mailing address. This form can be submitted to the Assessor's Office in person, by mail or fax (732) 826-1160.
